Hypertrophic osteoarthropathy (HOA) is a syndrome of clubbing of the digits, periostitis of long bones, and arthritis; HOA may be primary or secondary to other underlying disease processes; Secondary hypertrophic osteoarthropathy; Hypertrophic osteoarthropathy; Secondary HOA typically occurs in adults and is associated with other illnesses including: Pulmonary: Bronchogenic carcinoma, lung abscess,bronchiectasis, cystic fibrosis, pulmonary fibrosis, mesothelioma, sarcoidosis; Gastrointestinal: Esophageal carcinoma, colon cancer,inflammatory bowel disease (Crohn"s disease, ulcerative colitis), hepatocellular carcinoma, liver cirrhosis, amebiasis; Cardiac: Infective endocarditis, right-to-left cardiac shunts, aortic aneurysm; Thymoma; Lymphoma ; Connective tissue diseases; signs and symptoms include:Joint pain and swelling; Decreased use of the fingers and hands; Facial changes, coarse facial skin grooves; Thickening of the arms and legs; Oily skin, diaphoresis, gynecomastia, and acne; Secondary HOA patients may present with clinical symptoms before the underlying disorder can be detected. Signs and symptoms are similar to the above mentioned in addition to findings related to the underlying disease; (eg, bronchogenic carcinoma, infective endocarditis); [Ferri Clinical advisor 2005];-----------------------------------------------Hypertrophic osteoarthropathy is divided into primary and secondary forms; Pachydermoperiostosis (PDP), the primary form, accounts for 5% of all cases of hypertrophic osteoarthropathy; Secondary hypertrophic osteoarthropathy,also called pulmonary hypertrophic osteoarthropathy, is associated with underlying cardiopulmonary diseases and malignancies; characterized by digital clubbing and subperiosteal new bone formation associated with pain, polyarthritis, cutis verticis gyrata, seborrhea, and hyperhidrosis; Pachydermia of the facial skin and/or scalp) and periostitis, PDP is more common in African Americans than in whites; The male-to-female case ratio is approximately 7:1;
