Iron overdose with elemental iron tablets 60-250/kg of elemental dose (60 mgm elemental/230 mgm ferrous sulfate tab "3 grams/lethal") ;usually pediatric ; xray shows the pills; first gi bleed and ulceration, then transferrin capacity overwhelmed with severe acidosis, shock, myocardial and direct endothelial action/suppressant; lavage, hemodialysis, whole bowel irrigation and deferoxamine are the treatments; look for late bowel obstruction; hyperthermia, cirrhosis, coagulopathy, anuria, chf, seizures can occur; chelation-iron complex lung disease; iron overdose is commonest lethal overdose in children; exchanged transfusion has been used
