Adhesions and inflammatory response of arachnoid/dura in low lumbar spine with encasing sacral/lumbar neuropathy; often but not always caused by disk surgery complications; chronic condition. Low grade infection, trauma, or bleeding irritation or foreign body (talc) suggested. Often no proof of strict reason but chronic pain often with lumbar radiculopathy-like pattern and epicritic pain is evident. MRI is positive for areas of thickening and encasement of lumbar radicular area. usually quite painful and not easily repaired.
